Patient Information
Since we value you your time as much as we value your skin, please review the following information prior to your visit so we can focus our time on your skin care needs. We encourage you to fill out and print the patient documents highlighted below and bring them to your visit to help expedite the check-in process.

We are constantly evaluating new insurance programs, so please call to inquire if you do not see your plan. We will do our best to remind you of your appointment within 2-3 days of your visit.
Your Visit
With thousands of new skin cancers diagnosed each year, Dr. George thinks it is important to do a full body scan to evaluate your entire skin health. There are countless patients who come in for acne and have a skin cancer diagnosed. Full body evaluations will be performed on all new patients and on a yearly basis for established patients.
Please bring your pharmacy information with you. In order to decrease errors and save paper we have adopted an electronic prescribing system.
Note: Minors under the age of 18 must be accompanied by an adult for their first visit.
Financial Information
Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurances and non-covered services. Insurance coverage is a variable and we cannot guarantee what services will be covered by your particular plan. If you have an HMO type of insurance plan, then you will need to get a referral from your family doctor or primary care provider. It is your responsibility to know the requirements of your insurance program. We will ask you to sign an authorization form requesting that insurance payments be sent directly to our office when appropriate.
Co-Payments are due at the Time of Service
For your convenience; we accept personal checks, Visa, MasterCard, and cash.
A no-show fee of $50.00 will be charged to patients who fail to show for their appointment and who give less than 24-hours notice of cancellation.
Pathology, Labs and X-rays
Surgical specimens are sent to a lab to be analyzed. The fee for this is a separate charge from the office visit and may be billed directly by the processing lab. Results are usually available 7-10 days after the test has been performed. Questions regarding payments for laboratory services should be discussed with the lab.
Prescription Refills
Prescription refill requests will be taken only during
regular office hours. Hours of operation are 8am-5pm (M-TH), 8am-1:30pm (F).
