There’s a lot of noise online about dermal fillers—migrating product, stretched skin, “pillow face,” and fears that fillers will somehow “ruin” your face forever.

Meet Your Expert Injector: Dr. Rosalyn George

If you’re going to let someone put filler in your face, their training and experience matter. A lot.

Dr. Rosalyn George is:

  • A board-certified dermatologist
  • Founder of Wilmington Dermatology Center, established over 17 years ago

When Dr. George talks about filler safety, longevity, and technique, it’s coming from decades of hands-on experience and data, not TikTok hot takes.

What Are Dermal Fillers, Really?

Let’s clear up the basics.

Most dermal fillers used at WDC are hyaluronic acid (HA) gels. Hyaluronic acid is a naturally occurring substance in your skin that helps keep it soft, hydrated, and plump. As we age, we lose volume and structure—fillers help restore or enhance that.

At Wilmington Dermatology Center, we use three main categories:

1. Hyaluronic Acid Fillers (Traditional “Fillers”)

These are the gels that:

  • Restore lost volume in areas like cheeks, temples, lips, jawline, and folds
  • Smooth fine lines and deeper wrinkles
  • Subtly shape and define features like the chin and jaw

They’re designed to integrate with your tissue and move with your natural facial expressions. Think: support, not distortion.

2. Bio-Stimulatory Fillers

Examples include Sculptra® and Radiesse®.

Instead of acting purely like a gel “filler,” these products stimulate your own collagen production over time. They work especially well for:

  • Global facial rejuvenation
  • Skin thinning and laxity
  • Areas where structure and collagen support are needed long-term

Radiesse can act as both a filler and a bio-stimulatory agent depending on how it’s used.

3. Skin Boosters

These are very light hyaluronic acid formulations placed more superficially in the skin to improve:

  • Hydration
  • Glow
  • Overall texture and quality

They’re excellent for patients who want that “lit-from-within” look rather than big structural changes.

Fillers vs. Botox: Not the Same Thing

This comes up constantly in consultations:

“Can I get Botox in my lips to make them fuller?”

Short answer: Botox is not a filler.

  • Neuromodulators (like Botox® and Dysport®) are used for muscles that move. They soften expression lines by reducing muscle contraction.
  • Fillers address volume, structure, and support. They replace or add volume, fill folds, and enhance shape.

Fillers won’t stop movement. Botox won’t replace volume. Most great anti-aging plans include both—plus skincare, lasers, and sometimes tightening devices.

Where Are Dermal Fillers Used?

When done well, filler is often invisible—it just looks like you, on a really good day.

Common treatment areas include:

  • Temples – powerful but often overlooked area for restoring youthful structure
  • Under eyes (tear troughs) – very advanced area, only appropriate for a small subset of patients
  • Cheeks – lift and support, often improving folds without injecting directly into them
  • Chin & jawline – balance the profile, sharpen jaw definition
  • Nasolabial folds & marionette lines – soften the “parentheses” and “sad mouth” look
  • Lips & lip lines – from subtle refinement to more noticeable volume
  • Neck, chest, and hands – soften wrinkles and restore youthful fullness

Fillers are gradually degraded over time by your body. How long they last depends on:

  • The product
  • The area treated
  • The way it was placed
  • Your own metabolism and ongoing aging

One key point from Dr. George: once your “tank” is filled and balanced, you usually need less filler over time, not more.

Myths vs. Reality: What Fillers Do (and Don’t) Do

Myth 1: “Fillers are unsafe.”

Reality:
Dermal fillers have been studied for over 20 years with a strong safety record when used correctly. Complications are rare, and there are established protocols for managing them. The biggest safety variable isn’t the product—it’s the person injecting it.

Myth 2: “If I get fillers, I can never have a facelift.”

Reality:
Not true. Plastic surgeons commonly operate on patients who’ve had filler. Filler may still be present at the time of surgery, but it does not prevent a facelift, nor does it automatically make the results worse.

Myth 3: “Fillers migrate all over your face.”

Reality:
Fillers don’t just decide to pack a bag and move from your lips to your temples.

What people call “migration” is usually:

  • Overfilling a tight, closed space (like the lips)
  • Poor placement or technique
  • Product shifting slightly within the treated area over time due to movement and anatomy

This is why skill, restraint, and anatomy knowledge are non-negotiable.

 

Myth 4: “Fillers stretch the skin.”

Reality:
A syringe of filler is roughly the size of a blueberry. It takes a tremendous amount of volume to truly stretch skin, especially when small amounts are placed strategically and slowly.

When filler is dissolved—whether for a rare complication or just to tweak a result—the skin does not suddenly collapse or look “ruined.”

Myth 5: “All fillers are the same.”

Reality:
Not even close. Even within HA fillers, products vary in:

  • Firmness
  • Flexibility
  • Lift capacity
  • How they integrate into different tissue planes

Dr. George picks products based on area, goal, and anatomy, not just what’s on sale that month.

What to Expect: Before, During & After Filler

Before Your Appointment

To set yourself up for the smoothest experience:

  • Avoid aspirin, NSAIDs, vitamin E, fish oil, ginkgo, ginger, garlic and other blood thinners (if medically safe for you) for about 14 days before treatment.
  • Tell the team if you have a history of cold sores—you may be prescribed medication beforehand, especially for lip treatments.
  • Eat beforehand and stay hydrated to reduce the risk of feeling lightheaded.
  • If possible, come with a clean face (no makeup), or be prepared to cleanse thoroughly in the office.

During Treatment

At WDC, filler appointments are treated like what they are: medical procedures, not spa facials.

You can expect:

  • Pre-treatment photos to track your results
  • Careful cleansing of the skin (often more than once)
  • Optional topical numbing
  • Use of needles and/or cannulas depending on area and technique
  • Gentle molding and massage of the product
  • Clear explanations of what’s happening and why

You’ll see immediate improvement, but the final result takes a couple of weeks as swelling subsides and the product integrates.

After Treatment

Post-care is simple but important:

  • Avoid touching or massaging the treated area unless instructed
  • Skip makeup for a few hours
  • Avoid heat, strenuous exercise, and alcohol for 24–48 hours to minimize swelling and bruising
  • Use ice and cold compresses as needed
  • Expect to feel the product for a few days; this fades as it settles

Dr. George emphasizes a “marathon, not a sprint” mindset. Complex aging patterns are rarely fixed in one visit. The goal is gradual, natural progress, not a one-and-done overhaul.
